Educational Qualification
- Laboratory Technician: Higher Secondary or equivalent exam with Science stream; Diploma in Medical Laboratory Technology recognized by Government of West Bengal.
- Pharmacist: Two years Diploma in Pharmacy (D-Pharma) (Allopathic) from institute approved by Pharmacy Council of India (PCI)/AICTE. Proficiency in local language required.
- Staff Nurse: GNM from Institute recognized by Indian Nursing Council. Proficiency in local language required.

- Laboratory Technician: Consolidated remuneration of Rs. 9380/- per month.
- Pharmacist: Consolidated remuneration of Rs. 16860/- per month.

- Bio-data mentioning post applied, name, father's/husband's name, present address, permanent address, phone numbers, date of birth, age as on 01.01.2015, educational qualifications with board/university, year, subject, percentage/grade, experience details and declaration of correctness.
- Photo identity proof (Voter ID, Passport, PAN Card - original and attested copies).
- All original testimonials/documents and attested copies.
- One attested photograph to be pasted on top right corner of the bio-data and one to be submitted with application.
